Do not use this Site for a medical emergency. PT Hawaii is an outpatient clinic and does not provide emergency services. Call 911 or seek care at the nearest emergency department. Messages submitted through this Site may not be reviewed promptly.

Welcome to the PT Hawaii website (“Site”). By accessing or using this Site, you agree to be bound by these Terms of Use. If you do not agree, please do not use this Site.

1. Use of the Site

This Site is provided for general informational purposes about PT Hawaii’s services. You agree to use the Site only for lawful purposes and in a manner that does not infringe the rights of others or restrict and interfere with their use and enjoyment of the Site.

2. Not Medical Advice

The content on this Site is intended for informational purposes only and does not constitute medical advice, diagnosis, or treatment recommendations. Always seek the guidance of a qualified healthcare provider with any questions you may have regarding a medical condition. Do not make healthcare decisions, disregard any medical advice from a qualified resource, or delay seeking professional medical advice because of information you have read on this Site.

3. No Provider-Patient Relationship

Browsing and/or use of this Site, or submitting general inquiry or requests does not create a provider-patient relationship between you and PT Hawaii or any of its providers. A clinical provider-patient relationship is only established upon acceptance of care by PT Hawaii, mutual agreement to participate in care, completion of a formal intake process and initiation of care via a formal patient visit. If you are already an established patient of PT Hawaii, these Terms do not alter the existing patient relationship, standard of care, continuity obligations, or patient rights.

4. Appointments and Electronic Communications

An online appointment request is not confirmed until PT Hawaii confirms it. Site messages, general email, and public forms may not be monitored continuously and must not be used for emergencies or urgent clinical needs.

5. Accessibility, Nondiscrimination, and Language Assistance

PT Hawaii provides access to its Site and services consistent with applicable federal and Hawaii civil rights law. PT Hawaii does not unlawfully discriminate in access to its Site or services on the basis of race, color, national origin or ancestry, religion, sex, pregnancy or related conditions, sexual orientation, gender identity or expression, age, disability or any other status protected by applicable law.  

If a website barrier affects a non-urgent healthcare need, contact PT Hawaii at (808) 680-9123. For urgent needs or an emergency, call 911 or go to your nearest Emergency Room. 

6. Provider, Service and Location Information

PT Hawaii works to keep provider names, credentials, outpatient services, locations, hours and contact information accurate, but that information may change. Site content does not guarantee provider availability, eligibility for a particular service, or any clinical result. Contact PT Hawaii to verify current information. PT Hawaii reviews and corrects verified material Site inaccuracies.

7. Information You Submit Through the Site
The Site includes forms on the Contact page, the “Share Your Comeback Story” page, and the Employment page. By submitting a form, you confirm that the information is accurate and that you are authorized to submit it, and you consent to us collecting, storing, and using it to respond to you, to carry out the purpose of the form (described below), and to operate our business. We may share it with our staff and with service providers who help us operate the Site and our business under confidentiality obligations, and when required by law. We do not sell your information. Our Privacy Policy explains more.

7.1 Contact Form. Use this form for general questions and scheduling requests. Please do not include health information, diagnoses, insurance details, or other sensitive personal information in the form. To discuss your care, call your clinic at (808) 680-9123. Submitting the form does not establish a patient relationship, and the form is not monitored for emergencies.

7.3 Employment Inquiries. Information you submit through the Employment form, or send to info@pthawaii.com, such as your name, contact details, student affiliation, and resume, will be used to evaluate your interest, contact you, and manage our recruiting. Submitting it does not create an offer of employment or guarantee a response.

8. Recovery Stories and Testimonials

• Submission is not publication. We welcome your story, but submitting it does not authorize us to publish it. We will not publish your name, photo, video, or story without first obtaining your signed media release.
• Your story may be reviewed internally. By submitting, you permit us to read and evaluate your submission and to contact you about it.
• Accuracy and content. Please share only your own experience. Do not include other people’s names or private information. We may decline to use any submission.
• Minors. You must be 18 or older to submit a story. If the story is about a minor, a parent or legal guardian must submit it.
• Individual results vary. Stories and testimonials reflect individual experiences and are not a guarantee of results.

4. Intellectual Property
All content on this Site, including text, graphics, logos, and images, is the property of PT Hawaii or its content suppliers and is protected by applicable copyright and trademark laws. You may not reproduce, distribute, or create derivative works without our express written permission.

5. Prohibited Conduct
You may not: (a) violate law or another person’s rights; (b) attempt unauthorized access to any system, network or data; (c) introduce malicious code or disrupt Site operation; (d) impersonate another person or misrepresent your authority; (e)use the Site to send unlawful, deceptive, or unsolicited communications; (f) conduct security testing without written authorization; or (g) use automated means to extract or copy Site content at scale, except for search engine indexing or other data expressly made available for automated access.

6. Third-Party Links
This Site may contain links to third-party websites for your convenience. PT Hawaii does not endorse, control, or assume responsibility for the content or practices of any third-party sites. Accessing those links is at your own risk.

7. Disclaimer of Warranties

This Site is provided “as is” without warranties of any kind, either express or implied. PT Hawaii does not warrant that the Site will be uninterrupted, error-free, or free of viruses or other harmful components.

8. Limitation of Liability

To the fullest extent permitted by law, PT Hawaii shall not be liable for any indirect, incidental, special, or consequential damages arising from your use of, or inability to use, this Site or its content.

9. Governing Law
These Terms of Use are governed by the Federal and the State of Hawaii laws, without regard to its conflict of law provisions. Any disputes arising under these terms shall be subject to the exclusive jurisdiction of the courts located in Honolulu, Hawaii.

10. Changes to These Terms
We reserve the right to update these Terms of Use at any time. Changes will be posted on this page with a revised effective date. Your continued use of the Site following any changes constitutes acceptance of those changes.
